Yep, well either way i just finished my first run-through. Got to say ending was quite disappointing, at least the good karma one was.
Going for my second run-through, might attempt the alternate endings now. I still want to know what really caused it.

did you enter the vault via the front entrance thats bathed in ridiculous amounts of radiation? i went through that part right up to the door, but didn't want to enter that way. so i went back out, and turned TGM off again. no problems after that.
in there i got about 1300 rads/sec with radx and my radiation suit on. without those, the highest count was about 3000 rads/sec. that should kill you in less than a second, no matter what, if you don't have god mode on.
but then again: it didn't screw up my game, so i don't think that this area alone could do that to your character.
the area inside where that "special suitcase" is stored is radioactive as well. but it's not as loadet as the area outside, and it's quite survivable. i wouldn't want to camp there, but rad-x and a descent radiation-suit helps you survive long enough to get in, geht the case and head back out again, without irradiating you that much, so you don't need any electrical light at night to read books.

I entered only the front entrance (not lamplight) where i had over 3000 rad/sec without any suit to combat rad damage. I never entered to get the suitcase though, i saved Fawkes instead.

this darn hoster doesn't let me download Joshstal's savegame. allways tells me to come back later.
maybe you REALLY arn't supposed to enter the vault that way. i may have a look at it, next time i come that way.
if you have a savegame from before you entered that vault, try to load it and see if you do have that problem allready, or if it occures only after you entered the vault. might help. =)

I uploaded a save game, if it isn't working ask me, i could just host it on another website.
Anyways, i am on my second run-through already, and so far so good, i haven't enabled tgm, nor the trainers god mode, and everything seems to be fine, i basically made myself into god mode with 50k hp. To completely prevent from anything like that occurring again.

Yea the save from before that point works fine.
I was only using the trainer when I entered the area above vault 87 where the suitcase is. Taking about 600rad/s until I was sick with -10000HP. Since that point I wasn't able to fast travel and would die instantly. I'm guessing that if I had been using god mode instead this wouldn't have happened.
It's really not a huge issue, but it would be interesting to find out what made this happen.
How would I go about editing my save file, or is that even possible?

so you used the trainers god-mode instead of the ingame console-command? that may be the reason for this to happen. the ingame command prevents you from picking up any radiation. the trainer however seems to only freeze your health on some high value, leaving the irradiation of your character unchanged. this way you seem to have picked up a leathal dose of radiation. maybe taking a hand full of rad-away might help. (console code for radaway is 15167)
until now i havn't heard anything about the possibility to edit savegames. personally i'm still convinced, that the console is about the most mighty tool to edit the game to your liking.

The console is truly the best way, but unlmtd ammo, and certain things can't exactly be acquired through the console, at least not to my knowledge.
And yes at that time i had been using the trainers god mode, not the console, and i got like 40 messages saying i was struck with rad poisoning.
It took about 30 mins for all the messages to go away as well, your right it could have been that, but rad-away didn't help, i literally bought/consoled max, and used all of them, but nothing happened. And it continued to kill me when i took off god mode.
