Dude that sucks. Wish I could tell you a way to fix it but im at a loss on this one. The only thing I can offer you might be a possible explanation. Did you leave the card on top of your tv or something like that because the same thing happened to me a couple of years ago and that's where I found it last?

I thought it would be safest in the slot, so nothing would happen to it.. guess I was wrong..

That explains it then...You left it in there too long while playing..It overheated and Defragged it and it also needed to be cleaned ocasionaly as well...
