Not many trainers have the NEW in-game overlay, some have an early version of it which is probably what you are seeing.

You need to press Ctrl+Shift ingame first. This removes input from the game and enables input to the overlay. But it's also written on the trainer + on the overlay itself.
If there are no boxes to tick on the overlay, then it's an "old" overlay.
If pressing hotkeys results in "deactivated" then you have an "old" overlay version which is bugged. As a workaround uncheck "mute hotkeys" in "Ingame overlay" tab on the trainer BEFORE activating the trainer. If that also doesn't work you are using another version of the "old" overlay which is completely broken. In this case you need to tell me the game and trainer so I can update it.
