does this trainer works with the new DLC Highway patrol?

You would have to try it. Trainers are as a general notice made for base games only, so it may work and it may not work.

Some options still work, but most doesn't. Stop time and end shift work. Those 2 are at least good enough to play until it is updated.

is the highway patrol expansion a completely different game?

Does the options not already work from this trainer? Looks like it's just a DLC?

Or is it a new game?

According to the store page it's a dlc expansion. Also included in the gold edition of the game. They also just updated the game.

Almost all these "updates" and fixes are centered around the FPV "beta" aspect of the game, which I have no idea how long this "beta" is going to go on, but we may just put this title ON HOLD if it has turned back into an Early Access Beta experiment where they are going to patch it incessantly and break it.
