This update broke controller support. When I use trainer with controller my controller don't let me move forward but if I use trainer without controller it works fine. If I use controller without using the trainer at all the controller works fine in game.
Oh no, if only this issue has been mentioned a 1000 times, and if only we had a common issue post where the solution has been posted and referred to 1000 times

I activated the super damage and I'm unable to aim can't move the sight to aim at anything
I don't think this is an issue specific to the trainer. I've had this happen with the hacking, interactions, etc. I think there's something funky with the game code in general that causes it to bug out once in a while. Turn all options off, save, reload, it should work.

I used the trainer -- but now I cannot save, even when things are turned off. When I try to save manually, I receive an error that I cannot because V. "has flatlined." Has anybody else gotten this error or found a fix?

you probably have ghost mode activated, it clearly states in teh trainer that you catn save while it is on, the game cant even detect you, and thinks your a ghost...hence ghost mode.
Check your settings - set up a hotkey to disable all cheats.
