I used both CE and Cosmos and i could not change my attribute points and perk point during game. It work at character creation though.
You can improve your primary characteristics as you gain a level.
The best is to gain at least 2 levels in order to make it easier to find the address of the characteristic points.
For skill points it is possible, for example, to do a few small hacks at the beginning of the game to increase the skill. It is the easiest to make it go up with stealth. And when you have gained a few points you look for the address of the skill points.
