I played this game all the way through without the trainer, I only use trainers to mess around after completing the full game without a trainer first. And when I played the game without a trainer, I had all my settings on Ultra, and the game ran smooth and without any issues.
Once I installed the trainer, the game now runs coppy. It stutters, freezes for 10 or 15 seconds, and then starts again sometimes just going frame by frame for a few seconds, and then back to normal. And it does this constantly the entire time the trainer is running.
I went into the options, and turned all my settings down to high, same issue. So I set everything to medium. Same issue. And each time I ran the game in each setting level, getting the same issue, I turned the trainer off, and the game ran smooth once again.
Is anyone else experiencing this? Since it only happens when the trainer is running, I have to assume there's something in the trainer doing this. Any ideas?

I had this problem even before trainer so it wasn't trainer issue in my case. Fix was changing fps to permanent 30 and then the following: Open Nvidia Control Panel > Manage 3D Setting > Program Settings > select “Watch Dogs Legion” and make the following changes.
Anti-aliasing-FXAA – “Off” Anti-aliasing-Gamma Correction – “Off” CUDA GPUs – “All” Low Latency Mode – Ultra. Set Power Management to “Prefer Maximum Performance” Shader Cache – “On” only if you are using an HDD. Don’t need to enable it for an SSD. Set Texture filtering – Quality to “High Performance” Triple Buffering – “Off” Set Vertical Syn to “Use the 3D application setting”
This helped me with this issue. Not sure if it will help thou tho.
