Odd that I have not received this bug at all?

That bug seems to happen when you/the game closes with that cheat on, after you gain a health upgrade (either through the upgrades you pick up, or from the skill tree) it disappears.

Yes, but it resets to 4000 if you turn that option off and back on. That is incredibly annoying

Super jump is not working with me like few others have mentioned. However, rarely the trainer has a partial crash and unlimited force and unlimited lightsaber stamina meter stop working and give errors, but other options keep working and super jump starts to work when that happens.

When I restart the trainer and game however, everything except super jump works fine.

Whenever I open the trainer, I don't see the "Customized for" message, and the trainer just hangs.

Working fine here. Something on your end likely interfering with it.
