1: Can the trainer be used in single-player campaign (as in start the campaign and play through) or does SV_cheats need to still be set as 1 and the episodes have to be manually typed in via the console, like with the inbuilt cheat codes?
2: What is the risk of VAC Ban for using a trainer instead of the dev issued codes? (Like blizzard banning for StarCraft II trainer use, in Singleplayer mode instead of using dev issued codes).

Yes, the trainer should be safe to use in singleplayer. Iv been using it for years without problems. You dont need to do anything else other than the trainer, no console is needed. Just launch the trainer, start the game and have fun.

It should work for STEAM as well, been years since I played it but I'm pretty sure it worked. Really you have nothing to lose by trying and if it does need a STEAM version the request system is there to use.
