Open the trainer first (You can see this in the readme file) start up the game and either start a new game or load a previous save. Once in-game choose a place to infect and wait until you get at least one point towards DNA advancement.
Once you have at least one point of DNA you can then activate the trainer with the 'F1' key, you should hear "Trainer Activated", once you've done this you can alt-tab (Easier in windowed mode) and modify your options.
You can view every option easier in the readme file.
Have fun
P.S. Thanks 0x90 for the quick update, having lots of fun now
Edit; If you wish to slow down the cure advancement, with the trainer opened, find the option called "Cure Requirement Base Multiplier" and set it to 100, this will cause the cure to take nearly forever to complete, even on Mega-Brutal difficulty. [NOTE] Only use this when the cure starts developing (You can see this at the bottom right of your screen, indicated by the lab vial), otherwise it won't work.
For your DNA points look for "evo points", type in your desired number for your desired DNA points.

Okay, I'll try to keep this as simple yet as detailed as possible.
Before you run the game at all you must open the trainer first, otherwise you'll get that message, and believe me I've tried going around it.
If this isn't working there is the obvious action of adding the trainer and/or game to your anti-virus' exceptions list. But one that tends to be overlooked is the UAC.
So, UAC or User Account Control, I won't give you a massive explanation as to what this is or why it's important, instead I'll try to get you on your way to destroying the world because of it.
First things first, open your control panel and at the top-right of the control panel window there should be a search box. Inside this search box type in UAC and this should bring up two options, click the one under "Action Center".
Mine is set to default by... default, but yours could be higher or lower, you can try setting it higher or lower, with mine I get a confirmation box before I run the trainer.
It's a long-shot but if it works then you won't end up complaining about this ridiculously long post that was completely unnecessary.
~~~
For the not-so-into-reading people check your anti-virus exceptions etcetera, or open your control panel, search for UAC, mingle about with the slider.
~~~
If this doesn't work you can always wait for an administrator, moderator or an editor to give you a hand.
