Is that the one on the bridge with the two other enemies firing at you and the crappy camera angle and because of it you have no idea where the guy on the horse is? That ****ed me off a lot as well.
Silver coin challenges STILL **** me off so so so much that I have never completed Diddy Kong Racing.
In Starfox 64, there are those two planets in a row. The under-water one and then the one with Falco's friend Katt. I don't think I have ever completed the one with Katt due to me hating both levels so much. I usually go a different root and try to get to Sector Z through Bill.
The mutated rancor level in Star Wars: Jedi Academy. It is not necessary that you complete it, as you only need 4/5 missions to proced. But I have never completed it.
Tales of Symphonia had many many dungeons where I nearlly went **** IT! and just gave up. The one with the berries off the trees which you have to blow in to summon fish being the main one -_- But man, that game had some horrible dungeons.

Silver coin challenge was sooo friggin' hard man. I remember me and my buddies trying to beat everything in Time Attack mode to get Tic Tok or whatever his name was.
Fun times....but aggravating PIGGIE SORCERER!
There are so many games I can mention!
1) Mirror's Edge: Some huge gap between a building and you.
2) Half-Life Episode 2: I was unable to kill some huge ant like insect, it would'nt die!
3) Batman AA: There was a level where Joker will kill the hostages if you get detected.
4) Splinter Cell: There is not a single installment I have completed.
5) Prince of Persia: Except for the 2008 game, I have found the acrobatic movements (Wall runs and stuff) a little tough.
6) Assassins Creed 2: Stuck in a stealth mission where you are supposed to loot some chest surrounded by guards.
7) Resident Evil 4 for PC: Controls suck, the combinations don't give you any idea on whay key you have to press! So I was stuck for almost 2 years on the very first level (Where the villagers push a boulder over you) untill I found a key replacement mod.
8) Portal: Some puzzle on the second or third chapter.
9) Doom 3: If there wasn't a cheat called "noclip", It would have impossible for me to pass the first hell level (Not the first level in the game).
10) Need for Speed Shift and Prostreet: Drift races are almost impossible for me to win.
I also had that same problem with Resident Evil 4 for the PC!
The control scheme was utterly ridiculous! There was no way to figure out what keys to use!
