I've seen quite a few games now with this listed next to the title. Does this means that we only have to download this cheat once, then it will upgrade itself? If not, Can you explain exactly what it does mean. Thanks.

the trainer will try to work through patches of the game. sometimes the trainer will still need an update if the game changes too much

I think the autoupdating trainers don't update in the sense like you would update a program. Do you mean with update that the trainer can find the pointervalues on its own, and thus if the adress were to change in an update, it would find the new adress for the pointer through logic?
