I'm getting the same issue several folks have posted here recently;
Spoiler: while the trainer activates just fine, only option 3 (instantly exit game) seems to function at all. For the rest, even though the trainer activates, each of the other four functions (enhanced speed, reveal map, show monsters on map, and no-clip through monsters) fail to activate at all, that includes the trainer saying "activated" not happening.
This is my first time using the trainer with this game (though not my first time playing the game). I was just trying to spice up the game play a bit with one of you guys' wonderful trainers, so given that the trainer status is still currently active, might I humbly request you nice folks take a quick poke at it, at your leisure of course? Thank you kindly.

D3 version: 1.08.16603
Trainer Version: Diablo Trainer 1.08.16603

Well I managed to get it to work fairly reliably. Instead of activating it at the main menu, I wait until it's on the "character ready" screen (where you've already got your character selected and you're ready to hit play) and activate the trainer there. Then, when still in the town, I cycle through each option once (except the insta-exit game one, of course) and they'll all work.
Hope this helps other people who were having the same issue!
